[Note: In this document Indexer refers to Atlantic Authoring and Publisher refers to

PROJECT NAME: Index to ___________________________________________ (work)
SCOPE OF WORK: Indexer will provide an alphabetical, professional quality subject index to the
above title, a work of approximately_______pages. Text pages, illustrations, photographs, tables, and all
other pertinent material will be indexed. Additional indexes (cited author, etc,) can be prepared if desired
(See “Cost of Services” below.)
TIME REQUIRED: The final index will be delivered within ____ ( ) business days after receipt by the
Indexer of final page proofs, assuming these are received by____________(date). Should final page
proofs be delayed beyond___________ (date) or the pagination change, the turnaround time may be
increased.
PRODUCT DELIVERY: Cumulative index(es) copy will be in the following format (select one) □MS
Word for Windows, □PDF, □ASCII, or □RTF. File(s) will be sent (select one or more) via □e-mail
attachment, □on 3.5" floppy disk, □on CD, □a hard copy printout, □camera-ready copy (provide full
specifications: point size, leading, number of columns, margins, etc., in an attachment). NOTE: Delivery
costs for Copy submitted by a method other than e-mail attachment will be billed to the Publisher.
AUTHOR CREDIT & EDITORIAL APPROVAL: □ Publisher agrees to provide credit to the
Indexer either on the copyright page or on the first page of the index itself. □ Publisher will not
provide credit to the author of the index. □ Indexer agrees to review any editorial changes made to the
index and will be available for review of index page proofs during the time period ________________
(dates). □ Should differences result regarding editorial changes made to the index, the Publisher agrees
to remove credit for the index if so requested by the Indexer.
TERMINATION: Should the Publisher terminate this Agreement less than seven (7) days prior to
commencement of work, the Indexer shall be entitled upon written request to a 20% cancellation fee
based upon the maximum expected payment for work as specified under “Cost of Services.”
COST OF SERVICES: Fee for the index preparation will be: $____ per indexable page. Fees for
additional/supplemental indexes (cited author, etc.) are subject to a_______percent surcharge on the
basic per page rate. (Strike-out and initial this last sentence if not applicable.)
TERMS OF PAYMENT: A final Invoice will be submitted with the completed index. Payment of the
outstanding balance is due upon receipt; delinquent after 15 days. A late penalty of 10% of the total
index cost as specified in "Cost of Services" above will be charged the Publisher for payments received
more than 30 days after invoice date. Payment will be made by Check or PayPal (online to
Sara@AtlanticAuthoring.com) to Sara Lynn Eastler, Atlantic Authoring, 4803 S. Liverpool Cir., Aurora,
CO 80015-6697.
COPYRIGHT: The Indexer will retain Copyright to the index until full payment for the index is
received. Upon receipt of full payment Copyright will automatically transfer to the Publisher.
